Expired AP: Active H1, H4 -> not sure

I am on H1. I just got it renewed for 3 more years. My Wife and I have an approved labor, and 140. Our 485 is pending under EB2-employment. We also have an approved and renewed EAD till ~ Aug 2009. Our approved Advance parole expired Sep 10 2008.

My questions.
(1) Would I be able to apply for AP again when needed or will there be a problem as I did not re-apply before the first one expired?

(2) How long does AP normally take say in Texas center? How many times can I use the AP to enter the USA? How do I request additional copies if one is required oer visit?

(3) Last and most important, My Wife traveled to India from Apr to Jun 2008. She was on H4 (which would have expired Nov 2008) when she left and she entered on AP. Since our AP expired a few days back, what is her status now? When I applied for H1 renewal, should I have applied for H4 renewal as well. She is not working.

Please advice.

Btw, I have not changed my employer nor do I intend to until the entire process is complete.

3. Parolee. She needs AP to travel or if you extend her H4 and she gets a H4 stamp, she can travel on H4.
