H4 VISA - Nepalese Citizen

I am an Indian Citizen working in the the US on a H-1B visa. My fiance is a Nepalese Citizen. I need to apply for an H-4 Visa for her after we get married in a two months. My question is --

-- How do I apply for a H-4 Visa ?
-- Does she need to apply at the US Consulate in Nepal ?
-- She would not have time to Change her Maiden name on the Passport after marriage and we want to leave together for the US ? So Is it okay if her H-4 visa has her maiden name ? Can it be changed later ?

Please advise.

What do you mean "if she wants to keep her Nepalese Passport"? How will she get any other passport?

Orion What I meant was she is a Nepalese Citizen and thus a Nepalese Passport Holder. She went to School and College in India but home country and country of birth are Nepal. Her Parents are in Nepal.

Can she apply for an H-4 in the US Consulate in India or she has to apply for the H-4 at the US Consulate in Kathmandu ?

She can apply in India as she has been living there for years. Is she a documented foreigner in India? legally present? If so, she should not have a problem visiting a consulate in India.
