H1-b questions

Hi,

I am on an H1-B status now and it was my first visa i took 2 years ago and i have one more year before it expires in september of 2009, so my questions is can i apply for an extension here in the US or does it have to be in my home country only?

And it would be the same process of applying for an H1-B for the first time, or just some form of Filing an extension?

is it easy to get it approved or is it more of like a process that has no doubt of getting denied?
