Procedure for COS from H4 to H1

Hello All,

What is the Procedure to apply for a Change of Status (COS) from H4 to H1. My wife currently has an H4 I94 and her H1B is approved. She either needs to go for Visa outside USA or apply for a COS. Please advise me on the procedure and how long is it going to take before she gets her new H1 I94?
