Urgent - DS156- what shud be First and Last name if applicant has ONLY First name

Hi,

If an applicant has ONLY First name but NO surname i.e per passport:
Surname : (Note there is no surname)
Given Name(s): Archana

But fields 6 and 7 of form DS156 makes it mandatory for both Surname and First name to be written:

6. Surnames(As in Passport)
7. First and Middle Names (As in Passport)

So what should be value for 6. and 7. fields of form DS156. Please note that applicant has only first name which is Archana but NO surname.

I need to fill the form asap.... please help.

If you have a last name, get your passport fixed first.

If not, your name becomes the last name and first name becomes First Name Unknown(FNU)
