Urgent - H4 Child Visa appointment booking

Dear Immi Help User,
I have booked the appointment for my son (6 month old baby boy). While booking in online thru VFS Site. I have filled only DS156 and Petitioner details and Directed me to appointment booking screen. Its dosent show or allow me to fill DS157.

Could please any one advice me, Wether DS157 is required for child or not ?? or iam comming any mistake while filling the online forms ??
