DS 156 error in writing name/surname

Hi,

I did a mistake in writing the name/surname in DS 156 form.
I know those fields cannot be edited.

1) Should I just goto the Delhi consulate and ask the officer to rectify?

2) Fill another DS 156 form form US govt and take that?

or

3) Need a fresh application?

Any experience and help will be appreciated.
