Help needed on "Other Surnames Used" &"Who will pay for your trip" (DS 156) questions

Hi All,

I am planning to attend H1B Extension and fresh H4B for my family members at Chennai.

Help needed on following question of DS 156:

"Other Surnames Used" : Do I need to use my wife's family name for her application ? Recently she has got her passport in which her last name is my last name and her educational certificates are having her family last name.

"Who will pay for your trip" : Do I need to mention my current employer here (I am asking because when I first went for H1B stamping my HR mentioned it as our company).

Name and Telephone numbers of persons in US. Who you will be staying with or Visiting for Tourism or Business : What should I mention here for my application. For my wife's application I will be mentioning my name.

Yes, you have to mention the maiden name in the other names used section on DS156.

As per my opinion (if the below question is for ur Wife Application):

Who will pay for your trip : Do I need to mention my current employer here (I am asking because when I first went for H1B stamping my HR mentioned it as our company).

For this as correctly mentioned if ur company is paying for tickets etc..then ur company name but if not, Then it should be UR NAME and not SELF(As u should be paying for ur wife expenses as she is on H4 and dependent on u)

OR if the question is for ur application then SELF is correct (if ur company is not paying)

---- For you and your family the address tel.nos. and all other personal details will be your details. This means your address (even for yourself), your tel nos. and or cell numbers

---- If you do not have an address or a tel.no in the US, then you can think of alternatives

---- Understand the application is about you and your details. All information provided will be for your sake. No one else is going to entertain you unless you are on a visitor visa or some other kind of visa where all needs will have to be provided for you and your family
