urgent help please

I am a physician on B1, came to the US to complete medical exams and now I have been offered a research position by a teaching hospital on a H1 visa, starting April 6.

should i get H1 papers here itself or should i go back to my home country and apply with my consulate. I am confused which is better

My I 94 is good for another 2 months.

I am assuming its cap exempt. You can go either way, but recommended to goto home country as uscis frowns on COS from B visa and considers it fraud in some cases. When did u enter?

I entered on Feb 3 and have I 94 till May 3. I have previous visits to the US but always left before I 94 expired
