Question about DS 156 form

Hi ,

I was on H1 and now in India applying for H4 visa.I am filling DS 156 form and i have 2 questions.

1. The Q-26(How long to do you intend to say in US)
My husbands visa is valid till March next year and the sample form on immihelp says "Do not fill this more than 6 months"

What should i fill here
10 mnths according to visa validity or 6 months

2.Q-30(Have you ever been issued a US visa?Where?)
I am doubtful about the WHere part.
I was issued an H1 B visa through a company in Dallas and my centre for issuing visa was Vermont centre.After that i didnt leave the country .
So what place do i write there.

Please help

--- Anytime you go for visa stamping, most questions that are on DS 156 are regarding your visa stamping. Even for this question, they are asking about the visa stamp in your passport. So if you had an H1B stamping done in Mumbai (or Chennai, Hyderabad, Delhi, Kolkata etc), put the "Where" as Mumbai (or Chennai, Hyderabad, Delhi, Kolkata etc).

For the second question, I am still confused

I got my H4 from India(Chennai consulate) and then moved to United States.
I got my status converted to H1 there and i was on H1 till Apr of this yr (didnt go out of US till now)when my H1 got denied so i came back to India and now i am applying for H4.
So technically i didnt get any stamping done for my H1.
So can you tell me what do i fill in here.

--- Again, the question on DS 156 pertains to your existing visa "stamp" and in your case it is H4. So when replying to this question, your answer should be "Yes, H4 Visa, Chennai, India".

It does not matter how many times you have changed your visa "status" in the US. They are strictly asking you for the visa stamp in your passport.
