DS-156 Form

Dear Sir/Madam,

This is a request to guide about the DS 156 form, PARA 37:

I am Indian citizen and am currently on H1 visa. I am traveling to India with my family and for my return me and my wife need to get visa stamped. I have two daughters (3 and 1 year old) who are US citizen by birth and hold US passport and they are also traveling with us. I have no other relative here in US. I want to know what should I write in field 37 (about any person in US son/daughter whether citizen etc).
