H1b & H4 visa questions for filling out DS156

Hello,

I am applying for H1b visa and my fiancee (wife at the time of appointment) will be applying for H4 visa.

I will be getting appointment after my wedding in November.

Questions:

1) Do I get an appointment for myself and my wife at the same time? How does it work?

2) DS156 questions:

I am currently in the US. for Question 15 should I provide US address?

For question 16, should I provide India phone number or US phone number?

Question 17, since I am not married yet, but will be married at the time of appointment, should I provide status as Married or Single?

I think this will also dictate dependent visa.

1) While taking appointment for you, you can add dependents.
2) Indian address, phone #.
If you're adding your wife, you can put Married. Or for separate appointments, it doesn't matter.
