Hii..My name is Deepshikha mohan kumar arora as per my passport where surname = arora and given name is deepshikha mohan kumar. WHen I landed in US, I filled the I94 where I gave the family name = arora and First(given) name = Deepshikha since mohan kumar was not fitting in the I94 boxes. When I go to apply for SSN, will they generate the name as Deepshikha arora or Can I ask them to generate it by Surname = Arora , First name = Deepshikha and Middle name = mohan kumar. Do they generate it by I94? Is middle name field mandatory on SSN generation form when they verify the I94 since I94 doesnt have my middle name? Please suggest
