Query about H1 extension

Hi,
I got H1B workpermit, but got my H1 visa stamped after 1.5 years, Then I came to USA in January 2006. In 2007 I got my H1 extended up to June 2010.
I am with same employer during this period.
By 2010 June I will complete 4.5 years on H1. My query is -
Can I apply for one more extension of 1.5 years?
Can I apply H1 extension with same employer or Do I need to change employer?
