H4 Doc'ts to include spouse name

Hi Friends,my wife got H1b and she left too US,she is in US from last 3months,now she is sending papers to me i have to go by on H4,for this wat the doc'ts required and in my passport my spouse name is not there.is it neccessary to add it? i ahd marriage certificate and photos of our marriage.And what all other documents are required please help me.
