Documents required for COS from H1 b to H4

Hello All,

As per my knowledge, the list of documents required for COS from H1 B to H4 are
1) I-539 form, a check of $300
2) My pay stubs, I-797,LCA, Employment letter
3) My spouse's H1 B status, pay stubs
4) Our marraige certificate

Can anyone please let me know if I'm missing any document and also on what name the check should be made.

Would really appreciate a quick response.

Not sure if you meant that in addition to:

I-94 copy
Copy of all passport pages
