Tax for Resident Alien F1 to resident Alien H1B

Hi,

I came to US in 2004 on F1 visa and became resident Alien on Jan 1, 2009 due to 5 year rule. But one of my documents says that my resident alien change date was on March 1, 2009 and resident alien start date was on Jan 1, 2009.

1) What is the difference between these start and change dates?

I have changed my visa to H1B on April 1, 2009 and if I am considered here on a resident alien status effective from Jan 1, 2009 then :

2) am I under dual status alien? just want to make sure.
3) What kind of forms do i need to fill for tax because I changed from F1 to H1B (but considering the fact that I became resident alien on Jan 1, 2009) ?
