DS-160 Form Question

Hi,

I had some question while filling out the DS-160 form. I am on H1B and will be going to chennai to get my H1B restamped.

(1) Should I have to mention my name in Native Alphabet or can I just say Does not apply
(2) Home addreess - SHould this be India or US address.
(3) Also home and mobile phone number. Should I give my India or US number?
(4) My old passport was issues in Chennai and it expires July 2010. But I already have a new passport issues in Houston TX. So should I provide my new passport details or the old one?
(5) What is Passport book number? Since it asks for a passport number which I can find in my passport but not sure what the passport book number is ?
(6) Travel plans - I have already made travel plans. So I can provide my Arrival date back into the US but what do I put in for my departure date from US?
(7) My previous issued visa details - It asks for a visa number. Where do I find this number? The only number I see in my visa in a control number
(8) Do I have any relatives in the US. I have my cousing here but what is the advantage/ disadvantage of saying yes or no?
