H-1B to F-1

1) I am in H-1B Visa and planning to apply for a graduate degree for Fall semester. University wants me to complete ‘Sevis I-20 - Request Form for Graduate Student’. If I complete this form and later if I get an I-20, do I have to attend the university? In other words, does my H-1B automatically changes to F1 if I complete this form now?
2) I already have a Master’s degree in US and utilized the OPT Period. Now, I am switching to different area of study and applying for a Master’s degree. Do I get OPT again?
