H1 & H4 Visa Stamping

Hi,

My husband is on H1 visa currently. I was working on H1-B visa, lost my job in Sept and now I'm on H4 visa. Both of us need visa stamping on our passports (H1 for him and H4 for me). I have following questions:

1) Should both of us together for the visa interview?
2) DS 160 does not have category of H4 status
3) When I'm getting my HDFC receipt, do I have to indicate it is for H4?
