H1 renewal + new H4 in single DS 160 form

I need a help regarding filling DS 160 form.

1. Can i fill only one form for my h1b renewal plus for my wife H4 visa?
2. If possible,could you help me in guiding in filling the form.

you have to fill two different forms both for yourself and for your wife.
