I am currently on OPT. I have got full time employment with a company and they might sponsor my H1 visa later. One year of my OPT will be over in Jan 2011. I can extend it for 17 months. When should I ask them to file my H1 visa? Other question is: I was planning to go to India in December, while I am on OPT and have a job. So will that be a problem while entering back in US? What all documents I should carry while entering back in US?

1. It is really up to you to ask them when you want to go for H1 visa. It is best to continue OPT as much as you can.

Can I leave the country while I am on OPT?

Yes, as long as you have valid F1 visa stamp to reenter.
