Joint application H-1 and H-4

Hi

I am planning to go for visa for myself (H-1) and my spouse (H-4) in the same appointment. Here are my questions.

1. How do I make a joint DS-160 application for H-1 and H-4

2. At which point in the application will I be able to say that I need a joint application.

3. With the joint application will I get one bar code to fix appointment together or will I get two seperate bar codes.

4. Is there are any particular steps I have to follow while filing the joint application.
