Visa number & date of issuance - DS 160

I got married in 2007. I applied for H4 & have stamping in my passport. In 2009, i've extended my H4 visa. Now i've visited chennai, India and will be going for stamping. I'm filling out DS 160. I've to provide Visa number & date of issuance.

Visa Number: Should i give the visa number located in my passport? But i've extended my visa. My I-797A has only receipt no & I-94 number.Which number should i give?

Visa Issuance date: Only my visa in the passport has issuance date. But that visa has expired. My I-797A has only receipt date & notice date. Which date should i provide?

You are confusing yourself with visa and petition. The one that you extended is your petition, not your visa. Visa is the one that you get stamped in your passport by visiting a consulate. The questions clearly asks about the VISA.So look into your passport and answer accordingly.
