Affidavit for Child Born in INDIA

Hi All,

I am looking for a passport to my daughter currently in india. I am in US now.

I heard i should get affidavit(bonafide) from INDIAN EMBASSY@ US.

Could you please let me know, what i should get to Embassy office in person?

I heard just online form xeorx, passport of myself and address proof is enough.

Is that ok?

Its quiet urgent ... please help me out ...

---- Affidavit for what purposes. If your daughter is born in India, she will have an Indian Passport. If born in the US, a US Passport. What else do you need

For applying passport to my daughter in india i should send this affidavit to them(india) from US. They want proof of father's passport. For that indian embassy will provide bonafide after verfying my passport.
