My daughter's passport- no surname mentioned

In my daughter's passport, Surname is not mentioned. I am wondering how, when we provided that so. Kindly let me know if I can go ahead and fill the DS-160 form with First Name in both places for Surname and Firstname?

Or do I need to make changes in passport?

Best is to get it corrected. Becase passport is for many years.

As far as visa is concerned, embassy use whatever name is on pasport as Last(sir) Name. and for first name they use something - 'No First Name Given'.

I myself have blank sir name in passport. So embassy had used my middle name as my last name.

My mom has just first name is passport. I plan to stamp her visa in November. I think there should not be any problem.

I am not sure how much time it will take and the procedure to correcect the Passport.
Also my daughter will be on H4 and is just some months old so she would not need SSN or DL. So for now we will go with "1st name to be filled for surname" and 1st name as 'FNU'.
