I've doubt in Social Security No form.
What Should be my First Name, Middle and Last name?
If my Name as below,

In Passport and H1B Visa:

Surname : MURALI MOHAN (Father Name)
Given Name : RAJ MOHAN

IN I-94 :
Family Name : MOHAN
Given Name : RAJ

Please advice...Thanks
