Your H1B will only be extended if your employer files an extension request, and your employer can file for the extension at any time before your current H1B expires. If your employer does not file for an extension and you leave early, you will lose any unused days of your current H1B. The requirement that workers must remain in the U.S. for a minimum of one year after their approved Labor Certification Application is filed means that if you leave within 365 days of filing your LCA, you must return to the U.S. within 1 year later in order to not lose those full 365 missing days.
