How to apply for H4 Visa

Hi All,

I am currently in India. My prospective employer has filed H1 visa for me. It is already approved. He told me that he will be sending my approved document in India. And than I need to book appointment in consulate and get the visa stamped.
1) I need to travel with my family so when I asked about H4 visa, he told me that I can directly book appointment when I am going for H1 visa. Could you please let me know what is the correct process for filling H4 visa?

2) How can I check the minimum salary in US state?
