passport renewal

hello

my passport in expiring on jan 4th 2011 and i am getting renewed in person at new york. The website says there is an online application to be filled. do i fill in the application online or take a print out and fill it in and take it with me . and do i need to take an appointent to get my passport renewed?

For Chicago, I needed to only fill the application online, no paper application or print out.
