H1B visa renwal rule

My H1B visa expired in Feb 2009 but I have I797 approval till Oct 2012. Do I need to renew my visa within a year or I can be in USA without renewal until I leave the country?
Also how long does it take generally to get visa stamped if I go to Mexco, Canada or India?

You can remain until the validity of the I-94 without a visa.
