change from H1 to H4

Hi,

I had changed my status from F1 to H1 on October 2004. I was on training for 4 months , so does not have any pay stubs and now I want to change my status to H4 .I am currently in US and is visting India soon. So can I apply H4 from India ? If applying from US or India , should I submit any other forms than the regular H4 application
