I-94 departure record

I recently came back from US to India and my departure record I-94 came along with me and was not taken back in US.
To send it back i know the other formailites to be done but I need a sample application(cover letter) for this purpose. If some one already has it or has faced same situation and can provide me the details to be inputted in the covering letter then please pass it on.

You can just make a simple letter saying you did not submit your I-94 while leaving US. Therefore, I am submitting now.
