H1b - visa interview appointment question

While trying to book the visa appointment in vfs-us.co.in, i see the below question:

Have you been refused a visa twice within the last 12 months?*

Question:

1) H1b extension denials will be considered as refuse of visa?
2) If refused twice, is there any affect for the third time application?

Petition denial is not the same as Visa denial.
