DS-160 Question - Please help- urgent

I went to US on H4 and changed my status to F1 in US.
Now Im back in india and applying for a fresh H4.

In Ds160 form,
Have you ever been issued a U.S. Visa? YES

Date Last Visa was Issued:
(should this be my H4 visa issuance date which is on my Passport or the one in I-797 for F1)?

Are you applying for the same type of visa?

Are you applying in the same country where the visa above is issued and is this country your principal country of residence?

Have you been ten-printed?

Please help me with these questions.

Last date you entered the US and duration of total stay (approx in months or years)

Visa is only what you have in your passport. Change of status approval is not a Visa.
