Health Benefits to H1B employees

Hello All Employers, Employees:
We just started a IT consulting company and we are preparing our employee benefits package. Please let us know, if we hire and transfer H1B visa of our employee and would like to provide him/her health benefits how do we do that?
1. What are general trends of providing health benefits to consultants who work at projects in other states in US?
2. Do H1B or other consultants buy their own insurance? If so, how do companies (employers) provide them health benefits?
3. Are there any health insurance companies who take care of this?
