Visa Stamping renewal

I am sorry that If I am wasting your time.
I browsed the existing questions and got little confused. Please validate my understanding and respond for these simple questions.

I am going to H1B Visa renewal (Visa stamping) since my visa stamping got expired in sep 2010.Along with me my wife and son (8 years) also attending as there are on H4.We all have valid I797 till 2013 may month.
1. I understand that I need to have 3 DS160's and 3 HDFC receipts, is that correct?

2.When I am filling my DS160, I need add my wife and son as dependents, Will I have that option when I filling the 160? If not where will I have that option?

3. I need to specify an ” Indian citizen resident in US”, Correct?

4.I am Telugu and my passport belongs to Hyderabad. But current communication address is Bangalore. Do I need select "State of Residence:*" as Andhra Pradesh or Karnataka?

5. For question “Have you been refused a visa twice within the last 12 months?*”
My visa stamp is expired in Sep 2010 so I have to select "NO" , correct?

6.It is asking only my HDFC receipt details, When will I enter my wife and son HDFC receipt details, I am worrying that by the time I complete my form , my wife and son may not get the same interview date, How to handle this?

7. While filling the form, how many companies experience, Do I have to enter?

8.I heard that we need to send the documents 4 days before to consulate, is that same case to Hyderabad consulate?

9.I understand that I can take the appointment on any U.S consulate in India as I am going for H1 renewal (I am preferring HYD incase), is that correct?

10.I remember I filled two DS forms when I was going for fresh stamping 2007, Since it is Visa renewal Do I need only DS160?

>>> Yes. You need to select the "Family Option". Once yu complete your DS-160, you will be prompted to create DS-160 for your dependents.

>>> You can select that option for visa stamping.

>>> The state where your current permanant address in India is will be your state of residence.

>>> The question doesn't ask about an expired visa in the passport. It asks whether you ahev been refused a U.S visa by the consulate within the last 12 months.

>>> If you choose family option, then the primary applicant's HDFC receipt number is enough. Check with VFS helpdesk if yu have more questions.

>>> Whatever is asked in DS-160.

>>> For Hydrabad Consulate - YES.
