Need Info on Name Change Certificate.

Hello Everyone..

I am applying for H1B Visa. I have a slight difference in my surname in my Passport and Degree documents.. I heard that I would need to Provide Name change Certificate.

Can anyone tell .. what i need to do to make this Certificate. Whom i can contact for this. Do i need to make an affidavit for this.

I am kindly confused... as i need this to file urgently .. Request to please help!!..

It is advisable to get the name corrected in the passport as per your documents. If you are not in U.S, then ignore the information related to U.S (Like publishing in a daily newspaper in U.S etc). You should be able to get more information from the local passport office.
