    Hi,

    I need some help in filling up the DS160 form. If some one can help me out with the below questions would help me a lot.

    I am going for a first time H1 stamping. My employer is from Michigan and i work for a client in Illinois.

    1) U.S Taxpayer ID number.

    I have a SSN. But i am not sure whether i have a U.S. Taxpayer ID number or not. I filed tax for 3 years while being in F1 Visa and one year while being in H1 Visa.

    2) Home Address.

    Do i need to give the address in INDIA or in USA?

    3) Specific Travel Plans.

    I have already reserved the tickets. I am able to give the arrival date in US. But i am not sure to give which date for Departure.

    4) Temporary Work Visa

    Name of person/Company filed for petition ::: I mentioned my companys' (Employer) name.

    And then it asks for WHERE DO YOU INTEND TO WORK?
    Below it mentions NAME OF EMPLOYER and then the US ADDRESS.

    I am working at clients place in Illinois. My employer is in Michigan. Which address should i give now for this column?

    If i am to give my employer's address, then there is a section before where it asks, WHERE DO YOU INTEND TO STAY WHILE IN USA? i have given my local address in Illinois.


    Please help me out in this. Last question is the worst of all.

    >>> You can give Indian address where the consulate can contact you if required.

    >>> Give tentative departure date. Yuo can specify a date close to the petition expiry date as the departure date.

    >>> You need to specify the actual place where you will live and work. If it is Illinois, then that is what you have to mention here. Make sure your have an LCA approved for Illinois and the H1B is amended properly. It is illegal to work in Illinois with an LCA and H1B approved for Michigan.

      In the last question regarding WHERE DO YOU INTEND TO WORK?

      it asks Name of employer.

      Do i need to give my Clients name in there and the address below or My employers name followed by client address?
