H4 visa expiration date is wrong

Recently my wife got H4 visa and on visa expiration date is wrong. My petition is ends in Oct-2013 and visa expiration date is OCT-2016 and new visa is missing following information in ANOTATION compare of previous h4 visa stamp. 1. PN (Petitioner) 2. P# (Petition Number) and 3. PED (Petition end date).

All other information on visa is correct.

Please help me what to do? How can I correct the information? What can go wrong at POE? Please advice.
