Is 6yrs H1B extension CAP based on Visa Issuance or on First Arrival in USA

Does anybody knows that the 6 year limit for H1b visa holders starts from the Issuance of the first visa OR is it from the date of first arrival in USA

Only the time spent in the US in H1 or L1 status counts towards the 6 years.
