Sample of NOC for my Child's Application for H4 Visa

Hi,
This is Nataraj. I am in USA in H1-B Visa. My Wife and kid are in India. They are planning to go for H4 Visa.

I came to know that I need to provide No objection Certificate (NOC) from me to my wife for my Son's H4 Visa.

Please provide me the format or sample NOC if you have.
