H1 approved H4 pending, shifting companies

Hi ,
I am shifting companies from A to B. My H1B extension has been approved from A.
Now B said they will transfer it and the lawyers have prepared all the documents when they asked for my wife's H4 approval..
However, the H4 extension filed by A for my wife is still pending.

I conveyed this information to company B today. I am waiting on their response.
Both company A & B are big companies with 10,000+ employees.

Does anyone knows any issues or what happens under the above circumstances.
And yes B is doing premium processing.

If H4 extension is already been filed, then there is no need to file H4 again just because you are changing your employer. H4 is associated to your H1B status and not to your employer.
