H1B Extension time duration

Hi All,

My H1B visa is expiring on 30th Sept, 2012 and I have travelled to US as of now.

Considering that I would like to get my H1B extended from US, before how much time I need to be in US to apply for extension?

Also how much time does it take to get H1B extension after applying for the same? What are the precautions which I can take to avoid extension rejection?
