H4 doc lost

I have my h1b and wife's h4 valid until nov 2012.
H4 document which has i-94 at the bottom, lost for my wife. Infact we never got it. USCIS says they sent it But we never received, after some calls finally they said it might be lost, ofcourse we gave them prev address, that was also a problem.


Now if I want to transfer h1b from current employer to other employer, I think I need to put her H4 doc copy which has I-94at the bottiom right? How long it takes to get a copy? There is some process with 400 USD fee right? How long it takes to get it. We have a notice of action, 529A something, Can I use this Doc instead this one?

I recently got my H4 stamped and planning to go to US in April. As your were taking about I-94 document, want to know what it is.
Is that something will be given to me along with my passport stamped?. or something which will be there with my spouse in US. Is it a separate document or just stamp on passport?.

It is a card that will be issued to you when you clear the U.S immigration. I-94 will determine until when you can legally stay and work in U.S. The visa that you get at the consulate is only an entry documents to U.S.
