Full name change/declaration question

My fiance and I are about to get married. She is here on a B visa and we will be getting married in New York soon. Right after we get married we will apply directly for her green card. Her Chinese name is very difficult for English speakers to pronounce. We want her to use and English first name and my last name and we would like to use this name legally on a permanent basis. I know to change the last name is not very difficult to do, but we are talking the entire name here. What steps must we take to achieve this?

Since we have not applied for anything yet I would like to do it right as changing later could be much more work.
