Lost I-94

I have in truble right now. I lost my I-94 card last week.
I mean I've gotten stolen my information everything by someone. iOi
I need I-94 reissue as soon as possible, otherwise I can not go out to other country. even my country.
So if anyone who knows how to and process for applying reissue it. I'd appliciate.
This is kind of emergency. Please help me.
