Power of Attorney by mail

Hi All,

I have to apply for general power of attorney, for my location I have to send the required doc to SFO consulate.

My concerns are:

We are two applicant, do we need to two misc form, one for each application.

And as we have notarize the POA with two witness signed from state officer.

I am not clear about this, how to get it done.

Can anybody please guide me?
