Lost Passport and all the legal Original Documents

Hi Friends,

I lost all my original documents and passport. Can you please help me out how to proceed to get my passport.
Now, I don't have any original documents like I-20's, SSN, OPT cards to support my Passport replacement.

Contact your nearest consulate to get a replacement passport.
Contact your schools International Student department to get a replacement I-20.
Make an Infopass appointment to get a substitute I-94.
Then, go to the Social Security Administration office to get a replacement social security card.

Thank you very much for your quick response. I can get I-94 online right? and one more question is that I got my H1B effective from 10/01/2015. Do I still need to apply for replacement of my OPT?
