  • How to get copy of Vaccine Records from USCIS from 2006?

My wife immigrated to the U.S. from the Philippines 2006 on a fiancé visa. She was required to get many vaccines in Manila.

All records were put in a sealed envelope. This was then given to a U.S. Doctor who "processed" the paperwork and then provided additional vaccines.

How can we find out what vaccines she received in the Philippines. she is entering the medical profession and needs proof of vaccination.

Any help or guidance will be greatly appreciated.

Did your spouse need an I-693? If yes then you must feel out "Freedom of Information Request" form. There are many FOIA requests to choose from, but your request must go to USCIS.
    Here ies more info:
How to File a FOIA/PA Request
There are a number of immigration records available by request from USCIS; however, some records can only be obtained from the U.S. Department of State (DOS) or other DHS components such as Immigration and Customs Enforcement (ICE) or Customs and Border Protection (CBP). Our Submitting FOIA Requests table contains information that will help you file your FOIA request.

Before filing a FOIA/Privacy Act request with USCIS, please visit the USCIS Electronic Reading Room to ensure that we have not already proactively released the records you are requesting.

    Instructions for Requesting USCIS Records

Upon request of accessibility assistance, you will be contacted by a representative of our office who will address your needs. When requesting this assistance, please indicate your name, a phone number where you can be reached, and the FOIA Control Number in question.
